Consent to the Processing of Personal Data

I hereby grant EVLS FITNESS s.r.o., registered in the Commercial Register maintained by the Municipal Court in Prague under file number C 194137, with its registered office at Praha 10, Na Sychrově 975/8, Postal Code 101 00, Company ID No.: 242 97 909, the organizer of bodybuilding competitions, my consent to the processing of my personal data – first name and surname, residential address, telephone number and e-mail address – for the purposes stated below. My personal data will be processed for the purpose of providing services related to my registration and participation in the bodybuilding competition named EVLS PRAGUE PRO, which will take place on 27 August 2022 at the Hilton Prague Old Town hotel located at Praha 8, Pobřežní 311/1, Postal Code 186 00.

My personal data will also be provided to MUDr. František Zámola (Pro Tan Czech), with registered office at Jesenice u Prahy, Za Rybníkem 617, Postal Code 252 42, Company ID No.: 612 24 468. MUDr. Zámola’s website is available at www.protanczech.cz, for the purpose of offering body tanning services required for the competition and, in the case of female competitors only, for the purpose of offering make-up application services.

In the event that I win in one of the competition categories, my personal data will also be provided to NPC Worldwide, an organization based outside the European Economic Area in the United States of America, under whose auspices the competition is held. I have the right to request from EVLS FITNESS s.r.o., registered in the Commercial Register maintained by the Municipal Court in Prague under file number C 194137, with its registered office at Praha 10, Na Sychrově 975/8, Postal Code 101 00, Company ID No.: 242 97 909, as the organizer of the competition, access to personal data concerning me. I also have the right to have such data erased or, where applicable, to restrict its processing, the right to object to processing, as well as the right to data portability (these rights are described in more detail in Articles 15 to 21 of the GDPR). In connection with the processing of my personal data, I also have the right to lodge a complaint with the supervisory authority, which is the Office for Personal Data Protection, with its registered office at Pplk. Sochora 27, 170 00 Prague 7.

My personal data will be processed for the period necessary to fulfil the above-mentioned purpose. However, I have the right to withdraw my consent at any time. The withdrawal of consent does not affect the lawfulness of the processing of my personal data based on the consent given before its withdrawal. I may withdraw my consent, for example, by sending an e-mail to info@evls.cz, stating my first name, surname and residential address, together with the text “I withdraw my consent to the processing of personal data”.

I hereby grant EVLS FITNESS s.r.o., registered in the Commercial Register maintained by the Municipal Court in Prague under file number C 194137, with its registered office at Praha 10, Na Sychrově 975/8, Postal Code 101 00, Company ID No.: 242 97 909, operating, among other things, in the field of organizing bodybuilding competitions, my consent to receive commercial communications by electronic means concerning products and services provided by this company. In each individual message sent, I will have a clear and visible option to refuse this consent simply and free of charge.

At the same time, for the above-mentioned purpose, I hereby grant EVLS FITNESS s.r.o., registered in the Commercial Register maintained by the Municipal Court in Prague under file number C 194137, with its registered office at Praha 10, Na Sychrově 975/8, Postal Code 101 00, Company ID No.: 242 97 909, my consent to the processing of my personal data – first name and surname, residential address and e-mail address.
